Physical Therapy Marketing in Birmingham
Since June 1, 2024, Alabama patients can see a qualified physical therapist without a referral for up to 30 days or 11 visits, and cash-paying patients have direct access with no limit. That change opened new ways to reach patients directly. We help Birmingham physical therapy clinics use it, with search pages, campaigns and follow-up that also keep physician referrals flowing.
Direct access and rehab demand
What Act 2024-385 allows
To treat without a referral, a therapist needs a Doctorate in Physical Therapy, or a master's degree plus 10 years of clinical practice, and two extra hours of continuing education each year. If a patient shows no beneficial response within the limit, referral to another provider is required. Clinic marketing should say who qualifies and how the process works.
Clinics and competition
Census grouped 117 Jefferson County offices under therapy and audiology practices in 2023, with 50 more in Shelby County; 55 of those Jefferson practices were staffed by fewer than five people. Convenience drives the choice of clinic, so being visible in Hoover, Homewood, Trussville and the city's own neighborhoods decides a lot.
A rehabilitation hub
Encompass Health, headquartered in Birmingham, operates 179 inpatient rehabilitation hospitals and says about one in three U.S. inpatient rehab patients are treated in one of them. UAB recently opened a $156.7 million inpatient rehabilitation pavilion with 134 beds. Patients discharged from inpatient rehab may need outpatient therapy afterward, a natural audience for nearby clinics.
Growing a PT caseload
Explain direct access simply
Patients may not realize they can book without a referral. A clear page on Alabama's rules, limits and insurance considerations turns that awareness into appointments.
Keep referral partners close
Physician referrals still matter, especially beyond the 30-day or 11-visit limit. Regular updates and easy referral forms keep orthopedic and primary care offices sending patients.
Plans of care that stick
Reminders, exercise check-ins and progress messages help patients finish their plan of care instead of dropping off after a few visits.

Questions and answers
Not always, since June 1, 2024. Act 2024-385 lets a qualified therapist provide care with no referral for a maximum of 30 days or 11 visits, and patients paying cash have unrestricted direct access. If the patient is not improving, the therapist must refer them to another provider.
Yes. Referrals remain important for post-surgical patients, for care beyond the direct access limits and for patients whose insurance requires them. An easy online referral form and brief progress notes sent back to each referring office keep that channel healthy.
Build relationships with discharge planners, make it easy for families to find and book you online, and create pages for common post-hospital needs such as stroke recovery and joint replacement rehab.
